The BLET General Committee for Division 98 has negotiated an employee Medical and Dependent Care Expenses Flex Plan with BNSF that is separate from the BLET National Flex Plan. If you receive a booklet or application for the National Plan please disregard it.

We have a new plan provider this year. The provider is United Health Care. You can register for online access with UHC but it is a separate registration from your UHC Health Benefits registration.

Applications must be mailed by October 31st to be enrolled in the 2006 plan year.

The application link below is for reference only. If you were eligible to participate in this plan, UHC will have sent you an application by US Mail at this point. If you did not receive one and you felt you were eligible please contact your local Union Representative and or UHC for further information.
